Online Student Immunization Requirements

Online students at UL 老司机福利网 can choose to submit an immunization exemption if they do not plan to visit campus.  

You must either submit an , completed by your physician, or an exemption to register for courses.  

Note: All students must also complete and submit the tuberculosis questionnaire. See process below. 

To request an exemption:

  1. Log in to using your ULID and password. 
  2. Navigate to the 鈥淗ealth & Wellness鈥 card on your ULink dashboard. 
  3. Select "Medicat Patient Portal."
  4. In the Medicat Patient Portal, select "Forms."
  5. On the Forms page, select 鈥淩equest for Exemption from Immunizations Form.鈥
  6. Save the PDF to your device and fill out the form, selecting all immunizations.
    • Download  for free and use the "Fill & Sign" feature under "All tools" to complete the form.
  7. Return to the Medicat Patient Portal and select "Upload."
  8. Navigate to the drop-down box at the bottom of the page to select 鈥淰accine Exemption Request Form.鈥
  9. With 鈥淰accine Exemption Request Form鈥 chosen, click 鈥淪elect File鈥 to upload your completed exemption form.
  10. Submit the file and allow for 1-2 business days for processing before your hold is lifted. 

To submit your immunization records instead, follow these steps:  

  1. Log in to ULink using your ULID and password.
  2. Navigate to the 鈥淗ealth & Wellness鈥 card on your ULink dashboard.
  3. Select "Medicat Patient Portal."
  4. In the Medicat Patient Portal, select 鈥淯pload.鈥 
  5. Navigate to the drop-down box at the bottom of the page to select 鈥淚mmunization Record.鈥
  6. With 鈥淚mmunization Record鈥 chosen, click 鈥淪elect File鈥 to upload a photo or copy of your records. 
  7. Submit the file and allow for 1-2 business days for processing. 

To submit the tuberculosis questionnaire:

  1. From the top menu, select "Forms." 
  2. Select "TB Screen Questionnaire."
  3. Complete and submit the form.
